Benefits
Eligibility
All employees that work at least 30 hours per week and are classified as a full time employees in the payroll system are eligible on the first of the month following 30 days of employment (example: date of hire April 15th, eligible June 1st) except Basic Care Plan and 401K.
The Basic Care Plan - the initial eligibility is the first Monday of the month following 30 days of continuous employment as a full time employee.
Field hourly team members are eligible for the Definity plan after 12 months of active coverage in the Basic Plan.
401K - all employees that are 21 years of age or older are eligible to participate in the Pilot 401(k) Plan after 1,000 hours of employment.
Medical/Prescription Coverage
Basic Care Plan (provided through The American Worker) - The Basic Care Plan is available to any full time employee. This will be the only plan available to new hourly, full time employees for the first year. Hourly employees that are eligible (full time status and working an average of 30 hours per week) will have to complete 12 consecutive months of active coverage in the Basic Care Plan to be eligible to change to the Definity Plan. Hourly employees that waive the Basic Care Plan when first eligible as a new hire only qualify for the Basic Care Plan at the next open enrollment. This would also apply if coverage is waived after a status change from part-time to full-time, or a qualifying event that allows for benefits to be added. The Basic Care plan is an indemnity insurance plan that pays a fixed amount upon a claim submission. There are no pre-existing condition limitations. There are limits for medical services.
Definity Account Plan (provided through UnitedHealthcare)–is a consumer driven health care planthat reduces health spending by providing a financial incentive for consumers to choose the best healthcare value. The deductibles and out-of-pocket costs give patients an incentive to learn more about the cost and quality of care before spending. There are several components to the plan:
Health Reimbursement Accounts (HRAs) - Internal Revenue Service (IRS)-approved programs in which employers can fund an account that allows medical expenses to be paid
No office visit co-pays
Deductible - any required dollar amount that employee must pay each plan year for services received before the insurance plan pays any - $1,500 individual/$3,000 family deductible. Includes a Health Reimbursement Account (HRA) funded by Pilot of $600 individual/$1,100 family that counts towards deductible.
Co-insurance - costs for medical services split between employee (20%) and Pilot Flying J (80%)
In-network Preventive Care is covered at 100% - employee does not have to spend HRA dollars or meet any deductible to receive covered preventive care
Employee is protected against the financial risks of catastrophic claims with the co-insurance portion of the plan and an annual out-of-pocket maximum.
Prescription -There is no annual limit under either plan.
Definity – Generic drugs in tier I are available for $10, Brand Name/Preferred Brand in tier II for $30 and Non-Preferred Brand in tier III for $50
Basic Care Plan – This is a non-insurance program designed to save you money on prescription costs. The plan utilizes discounts and rebates available for the most commonly prescribed drugs to segment select brand and generic drugs into three price tiers. Select brand and generic drugs in tier I are available for $10 or less, tier II for $20 or less, and tier III for $50 or less
Dental Coverage
Provided through UnitedHealthcare (UHC) with annual benefit of $1,200. Plan pays the following for in-network charges:
- Diagnostic/Preventive – no deductible; the plan pays 100%; charges not applied to the annual maximum
- Routine - $50 deductible; the plan pays 80% after deductible
- Major - $50 deductible; the plan pays 50% after deductible
- Orthodontia - no deductible; the plan pays 50% up to $1,000 lifetime max
Vision Coverage
Provided through Vision Service Plan (VSP). Plan pays the following for in-network charges:
Premium Vision Plan
- Eye Exam - Every 12 months; $10 co-pay; plan pays 100% after co-pay
- Lenses - Every 24 months; $20 co-pay; plan pays 100% after co-pay
- Frames - Every 24 months; No co-pay; plan pays up to $120
- Medically Necessary Contacts - $10 co-pay; plan pays 100% after co-pay
- Elective Contacts - Every 24 months; No co-pay; plan pays up to $120
Premium Plus Vision Plan
- Eye Exam - Every 12 months; $10 co-pay; plan pays 100% after co-pay
- Lenses - Every 12 months; $20 co-pay; plan pays 100% after co-pay
- Frames - Every 24 months ;No co-pay; plan pays up to $130
- Medically Necessary Contacts - $10 co-pay; plan pays 100% after co-pay
- Elective Contacts - Every 12 months; No co-pay; plan pays up to $130
- Laser Vision Correction (Lasik surgery); plan pays up to $250 per eye
Healthcare and Dependent Care Flexible Spending Accounts
Provided through UHC for medical and dependent care expenses. An FSA is an employer sponsored plan that lets employees deduct dollars from their paycheck and put them into a special account that is protected from federal income, Social Security (FICA) and, in most cases, state income taxes. IRS regulations stipulate that the money in the FSA account must be used before a plan year is up; it is known as “use it or lose it” rule.A change in FSA election is allowed
during a plan year as a result of a qualifying event. FSA claims must be submitted on or before March 31st of every year for claims incurred in the previous year.
Short Term and Long Term Disability Coverage
Pilot Flying J recognizes the importance of financial well-being in the event of a disability. Most of us insure our homes, automobiles, and other assets, yet often overlook our most valuable asset – our ability to earn an income! Our regular monthly obligations such as mortgage or rent, utility bills, food, and other necessities continue even if we can’t work. For this reason, Pilot Flying J offers coverage in the event of a short or long term disability. It is important to note that Short Term Disability Insurance and Long Term Disability Insurance cover the employee only, do not cover dependents.
Short Term Disability
60% of base salary; 14 day elimination period, pays up to 50 weeks for team members without company paid LTD
60% of base salary; 14 day elimination period, pays up to 24 weeks for team members with company paid LTD
maximum weekly benefit - $1,500
Long Term Disability
60% of base salary; 365 days elimination period, pays up to age 65 for team members not receiving company paid LTD
60% of base salary; 180 days elimination period, pays up to age 65 for team members receiving company paid LTD
maximum monthly benefit - $6,000
Voluntary Term Life and AD&D Insurance
$10,000 increments up to 10 times base annual salary not to exceed $500,000.
Voluntary Whole Life Insurance for You and Your Dependents
Provides insurance coverage with a guaranteed benefit to meet long-term insurance needs.
Voluntary Accident Insurance
Covers the unexpected costs of accident expenses. This policy pays a specific benefit amount for expenses such as ambulance, ER, injuries, accidental death, etc.
Voluntary Critical Illness Insurance (including Cancer)
Covers several specific diseases, conditions, and procedures. There are a variety of benefits that can be designed to meet the needs of a family.
Company Paid Term Life Insurance and Accidental Death & Dismemberment (AD&D)
Company Paid Life and AD&D equivalent to one times base annual salary (rounded to the next highest $1,000). This is automatically added for all full time team members who meet eligibility.
401(k) Retirement Savings Plan
All employees that are 21 years of age or older are eligible to participate in the Pilot 401(k) Plan after 1,000 hours of employment.Pilot Flying J matches 60 cents for every dollar up to 6% of wages. Employee deferral vesting is always immediate; company match vesting schedule is as follows:
- 1 year - 0%
- 2 years - 20%
- 3 years - 40%
- 4 years - 60%
- 5 years - 80%
- 6 years - 100%
Educational Tuition Reimbursement
Receive up to $3,000 per year ($8,000 lifetime max). Eligibility criteria: work 30 hours per week or more, employed for 12 months of continuous full-time service, and continue to be employed by Pilot through the date you are reimbursed and for one year afterward.
The Learning Care Group Discount
Pilot Flying J team members are eligible to receive a discount on tuition for children ages 2 through 12 years from any of the Learning Care Group facilities: Tutor Time, Child Time, Children’s Courtyard and La Petite Academy.
Adoption Assistance
Reimbursement for adoption fees up to $8,000 per adopted child. Eligibility criteria: work at least 30 hours per week and have been employed by Pilot for one year.
